Click the above link to download the Registration Packet. **A Ticket must be Purchased in addition to filling out and submitting the Registration Packet in order to participate in the event.** 

Print out the registration packet and fill both pages out in their entirety. 

The packet can emailed to Info@tie1on4prostate-coloncancer.org, or faxed to 610-872-7061. 

If you are not able to fax or email, you can drop the completed packet off at:

Laborers' Local 413, 222 Penn St, Chester, PA 19013. 

Or bring with you on the day of the event.
